2. Upon insurance pre-authorization or financing eligibility completion, the Legacy Obesity Institute or SW Weight Management center will contact patient for initial consultation.

3. Patient must complete several clinician appointments with a registered dietitian, psychologist, physical therapist, internist or nurse practitioner. All clinicians must authorize approval before surgical consult can be scheduled.

4. OWLS (surgeon’s office) will contact patient to schedule initial consult with the patient’s preferred surgeon.

5. Surgery is scheduled and completed.

6. Lifetime follow-up.
