Step 2: Mail in your application/medical questionnaire and a copy of the front & back of your insurance card to:

Step 3: Insurance verification. Once an application is received, we will call your insurance company to verify if there is a bariatric surgical benefit.
If there is no benefit, a letter explaining this will be mailed to you, and we will help you look into financing options. Self-Pay Patients will meet with a Self-Pay Coordinator before being referred to Legacy for the welcome group and pre-surgery visits.
If there is a benefit, your application is given to our New Patient Coordinator and you will be contacted to start the pre-surgery process.
